What is needed to apply:
- Bond Request Form
- Questionnaire Form
- Certification and Authorization Letter signed by all owner(s) and spouse(s)
- Personal Financial Statement for all owner(s) and spouses(s)
- Previous 3 years of year-end financial statements
- Interim financial statement for the company
- Two accounts receivable aging reports – One for the previous year and one dated to correspond to the interim statement requested above
- Work On Hand Schedule
- Financial Institution Reference Letter—please complete the information requested at the very top and send the blank form to the banker for completion
- Relating to the project itself:
- The invitation for bids and form of the proposed contract
- The bid results, notice of award and contract, and any required bond forms.
After review of these initial underwriting items, we may need to circle back for additional information.
