  • Probate and Fiduciary Application
  • Personal Financial Statement
  • A copy of the court order signed by that judge that compelled the bond and stating the bond amount
  • Required bond if the court has one

What is Needed to Apply

  • Application signed and dated
  • Personal Financial Statement
  • Last two years of company year-end financials (if applicable)
  • Copy of the judgement and court documents that compelled the bond and stating bond amount
  • Copy of bond form that should be used – The attorney helping your client should be able to provide this
  • ***For Supersedeas/Appeal: A copy of the relevant statute from the state to verify whether recovery on the bond is limited to the penal sum listed or not — Again the attorney for your client should be able to provide this

ERISA Bond

The Act requires a bond limit equal to 10% of the maximum assets that the insured thinks will be held in the plan over the fiscal year (per the federal statute). The maximum bond limit required by federal statute is $500,000 (unless the U.S. Department of Labor advises the insured otherwise). If the bond amount needed is over $500,000, please advise before completing the attached application as a different document will be required.
